chore: 转移前备份 — cleanup + project book

- 删 VULNERABILITY_REPORT.md(已审完,合并到 commit 历史) - 删 middleware/ratelimit_test.go(单测延后) - .gitignore 加 *_test.go 规则(防误提交) - README XFF 验证段措辞更新 - 新增 PROJECT_BOOK.md(v0.1 待评审) 基线备份:目录迁移到非云盘路径前的快照。
